I, the parent/guardian, hereby acknowledge and agree to the following terms and conditions in consideration of my child's participation in the Nutcracker auditions.

I understand that participation in dance activities carries inherent risks, including but not limited to physical injury, and I/my child voluntarily assume all risks associated with participation.

I will not hold Happy Feet Ballet liable for any loss of personal items or physical injury, while participating in the Nutcracker Auditions.

Medical Treatment: In the event of an emergency, I give my consent to Happy Feet Ballet to secure medical assistance from any licensed hospital, physician or medical personnel, using any treatment deemed necessary for my child's immediate care and agree that I will be responsible for payment of any and all medical services rendered.

I acknowledge that I have read, understood, and voluntarily agree to this indemnity clause.
